Qualifications

• One year of home health case management experience preferred
• Minimum one year of nursing experience required; two+ years preferred
• Experience supervising or mentoring nurses highly preferred
• Current and unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver's license
• High level of clinical knowledge in home health care is key
• Self-directed and organized with outstanding interpersonal skills

Responsibilities

• Utilize clinical skills to create and execute patient care plans
• Oversee 2 nurses executing assigned care plans
• Conduct comprehensive patient assessments upon initial visits
• Develop advanced rehabilitation care plans based on assessments
• Work with patients and families to achieve health improvements
• Manage care episodes with interdisciplinary clinical teams
• Prepare clinical notes and charting in a compliant time frame
