How you'll make an impact in this role- Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient and family preferences and values.
- Monitor patient care through assessments, evaluations, and record reviews, serving as a resource for proper medical documentation, service appropriateness, and post-discharge treatment options.
- Advocate for necessary resources, eliminate care barriers, and maintain ongoing dialogue with supervisors and healthcare providers for effective plan implementation.
- Oversee patient progress and care quality while facilitating clear communication among all members of the healthcare team.
What minimum requirements you'll needLicensure / Certification / Registration:
- Required Credential(s):
• Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job
transfer date. - Preferred Credential(s):
• BLS Provider. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of
hire.
What additional preferences we're seeking- Previous in patient discharge experience highly desired
