Full Job Description
Our skilled nursing facility in Stevens, PA, is looking for a passionate and detail-oriented Registered Nurse Assessment Coordinator (RNAC) to join our team!

This full-time role offers an opportunity to make a meaningful impact by ensuring quality care through comprehensive resident assessments and care planning in alignment with state regulations and our facility's mission.

Salary: $90,000-$95,000 per year

Your Role and Responsibilities
As an RNAC, you will:
  • Conduct routine resident assessments according to schedules or as needed for condition changes, hospital discharges, and other events.
  • Accurately observe, assess, and communicate condition updates to the appropriate team members.
  • Collaborate with Social Services, Activities, and Dietary departments to ensure timely completion of assessments and CAAs with proper documentation.
  • Complete MDS sections, quarterly reviews, CAAs, and care plans as required.
  • Develop and update resident care plans based on assessment protocols.
  • Prepare and share the MDS schedule with the interdisciplinary team.
  • Transmit MDS data to the state weekly (or as needed) while maintaining meticulous records.

  • Proven experience in long-term care, with a strong background as an MDS assessment coordinator.
