Registered Nurse Assessment CoordinatorThe Allure Group is looking for an RN Assessment Coordinator to join the team at our Skilled Nursing Facility in Brooklyn, NY!
RN Assessment Coordinator Responsibilities- Conduct and coordinate the completion of MDS assessments
- Maintain and update all care plans and assessments
- Examine and review residents' medical history
- Inform other nursing staff personnel of any persistent issues
- Participate in the admission process while assessing prospective residents' nursing needs and appropriate placement
- Coordinate case management process
- Ensure that the facility adheres to all regulatory requirements and standards of practice in relation to the MDS care planning process
- Cooperate with inter-departmental personnel to ensure services can be adequately maintained to meet the needs of the residents
RN Assessment Coordinator Requirements- Valid, current RN license
- Previous experience in long-term care or skilled nursing facility
- One-year job-related experience in the MDS process
- Familiarity with various computer software
- COVID-19 Vaccination
