Key Responsibilities:
  • Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance.
  • Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care.
  • Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions.
  • Collect, assess, and evaluate patient health data to develop and implement care plans.
  • Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.
  • Utilize EMR and other systems to document care accurately and efficiently.
  • Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency.


Qualifications

Required Qualifications:

Preferred Qualifications:
  • BSN
  • Prior experience in endocrinology
  • Experience with patient follow-up, in-basket management, or outbound patient communication
  • Familiarity with diabetes and endocrine conditions (e.g., diabetes, thyroid disorders)
  • Knowledge of diabetes medications, including insulin therapies and common medication names
