Job DescriptionKey Responsibilities:- Conduct telephone and in-office triage, assessing patient symptoms, prioritizing care needs, and providing appropriate guidance.
- Collaborate closely with physicians and providers to support clinical decision-making and coordinate patient care.
- Communicate effectively with patients and families, including answering clinical messages, providing education, and relaying provider instructions.
- Collect, assess, and evaluate patient health data to develop and implement care plans.
- Ensure care is delivered in accordance with the NJ Nurse Practice Act and established protocols.
- Utilize EMR and other systems to document care accurately and efficiently.
- Participate in shared governance and contribute to process improvements focused on patient safety, quality, and efficiency.
QualificationsRequired Qualifications:- Graduate of an accredited Nursing Program and active NJ RN license
- Minimum 2 years of clinical nursing experience
- BLS certification
- Experience assessing patient concerns in a clinical setting (in-person, phone, or electronic communication)
- Ability to evaluate patient symptoms and escalate clinical issues appropriately
- Ability to review and identify abnormal or critical laboratory values
Preferred Qualifications:- BSN
- Prior experience in endocrinology
- Experience with patient follow-up, in-basket management, or outbound patient communication
- Familiarity with diabetes and endocrine conditions (e.g., diabetes, thyroid disorders)
- Knowledge of diabetes medications, including insulin therapies and common medication names
