Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year of clinical practical nursing experience, preferably in intensive care, rehabilitation, or medical surgical nursing.
• Home health experience preferred.
• Valid CPR certification required.
• Reliable transportation, current driver's license, and proper automobile insurance are necessary.

Responsibilities

• Provide nursing visits as assigned, including assessments and planning for patient care.
• Assess physical, psychological, social, and spiritual factors impacting patient needs and coordinate interventions.
• Initiate communication with physicians and interdisciplinary teams to ensure optimal patient care.
• Regularly assess patient needs to update care plans accordingly.
• Perform necessary nursing procedures as directed by the attending physician.
• Conduct clinical pronouncement of death and support families as applicable.
• Maintain accurate patient documentation and timely care plan preparation.
