Qualifications

• Graduate from an accredited registered nursing program and licensed in Illinois.
• At least one year of clinical practical nursing experience, preferably in intensive care or rehabilitation.
• Home health experience preferred.
• Valid CPR certification required.
• Must have reliable transportation and appropriate automobile insurance.

Responsibilities

• Provide nursing visits including assessment, admissions processing, and care planning.
• Assess physical, psychological, social, and spiritual factors impacting patient needs.
• Coordinate care by communicating with the physician and interdisciplinary team.
• Update patient care plans based on assessments during visits.
• Perform nursing procedures as directed by the attending physician.
• Facilitate clinical pronouncements of death and support families as needed.
• Maintain accurate and timely patient records and reports.
