Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year of clinical practical nursing experience, preferably in ICU, rehabilitation, or medical-surgical nursing.
• Home health experience is preferred.
• Valid CPR certification required.
• Reliable transportation and a current driver's license with appropriate automobile insurance are necessary.

Responsibilities

• Provide nursing visits as assigned by the Clinical Services Manager, covering all phases of the nursing process.
• Assess data from patients' physical, psychological, social, and spiritual aspects to identify needs.
• Communicate with physicians and team members to coordinate optimal patient care.
• Update patient needs and care plans through scheduled visits.
• Perform therapeutic, preventative, and rehabilitative nursing procedures as directed.
• Pronounce clinical death and support affected families when required.
• Maintain accurate and timely documentation of patient records and care plans.
