Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join The Office of Dr. Lev Barats, a well-established internal medicine practice focused on delivering comprehensive, patient-centered care for adults.

Position Overview

As an Advanced Practitioner at The Office of Dr. Lev Barats, you will provide adult primary care in a collaborative, team-based environment alongside an experienced physician and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and accessible care supported by integrated services and modern patient communication tools.

Responsibilities

  • Conduct comprehensive evaluations for adult patients and manage acute and chronic conditions

Competitive Salaries starting at $125,000

  • License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
    • Physician Assistant:
      • Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
      • Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
      • Current certification by National Commission on Certification of Physician Assistants.
      • At least one year of experience as a physician assistant in ambulatory health care.
      • Current CPR certification.
    • Nurse Practitioner:
      • Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
      • Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
      • Successful completion of pharmacology coursework acceptable to NYSED.
      • Three years of professional nursing experience in an ambulatory setting preferred.
      • At least one year of experience as a nurse practitioner in ambulatory health care.
      • Current CPR certification.
