The Psychogeriatric Resource Consultation Program of Toronto is funded by the Ontario's Strategy for Alzheimer's disease and related dementias and supports Behavioural Supports Ontario. The psychogeriatric resource program focuses on delivering knowledge-to-practice services to staff in long term care homes and the community who provide care to older adults living with cognitive changes and/or mental health conditions who may be experiencing challenging behaviours.

While based at St. Michael's Hospital, the Psychogeriatric Resource Consultant's (PRC) work will be focused in long-term care homes, community service agencies, and home and community care services in an assigned catchment area. Activities will include participation in in-services, behaviour support rounds/huddles, care-based discussion and coaching, as well as delivering curriculum.

The PRC will be required to provide services in accordance with the PRCP's Knowledge to Practice (KTP) framework leading to practice and workplace improvements within the serviced organizations and enhanced services integration. As well, the PRC will optimize opportunities to collaborate with other PRCs, demonstrate leadership in the care of the target population and participate in the program's accountability initiatives.

Duties & Responsibilities:

• Provides consultation services for individuals caring for seniors.
• Provides development and education leadership for caregivers to facilitate the provision of excellent care for vulnerable seniors.
• Contributes to program improvement initiatives.
• Educates caregivers on a continuous basis on appropriate care provision, to facilitate the identification of emerging care issues, etc., to ensure caregivers maintains knowledge and awareness of appropriate care provision for patients, understand when to make necessary adjustment to care to optimally benefit patients.
• Performs professional development responsibilities.
• Meets monthly with other Psychogeriatric Consultants to discuss issues that impact the discipline, challenges the role faces and potential solutions to problem(s);
• Performs administrative responsibilities.
• Develops reports, providing initial assessment of patient needs, following established timeline;
• Performs cross functional and other duties as assigned and/or requested.

Qualifications:

• Regulated Health Professional with Bachelors of Science Degree in Nursing / Health Disciplines required
• Master's degree would be considered an asset
• At least 2 years' experience in psychogeriatric services is preferred, otherwise significant experience in geriatrics, long-term or community based seniors care is essential
• Experience with standardized Behavioral Supports Ontario curricula such as, PIECES program, U-First!, and Gentle Persuasive Approach, is an asset;
• Demonstrated understanding of standardized psychogeriatric assessment tools
• Knowledge and experience in adult education
• Excellent counseling skills with strong interpersonal, facilitation and problem solving skills required;
• Demonstrated excellent facilitation, verbal and written communication skills required;
• Demonstrated commitment to ongoing learning and professional practice development required;
• Proven ability to deliver training and other programs to meet the needs of clients and family members required
• Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers, and other members of the community and public
• Able to work independently and coordinate work with colleagues and peers required;
• Dedicated enthusiasm for teaching and mentoring of staff and students required;
• Driver's license and the ability to travel across the region and the GTA on a daily basis
• Proficiency in the use of computing software such as, Word, PowerPoint, Email
• Knowledge of the consultation process an asset;
• Ability to converse in a second language an asset.
