What You'll Do- Conduct psychiatric evaluations and follow-up visits
- Provide medication management and treatment planning
- Support patients experiencing depression, anxiety, psychosis, mood disorders, and dementia-related behavioral challenges
- Partner with caregivers, families, and facility staff to improve patient outcomes
- Collaborate with an interdisciplinary team to coordinate comprehensive care
What We're Looking For- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
- Active Colorado NP license
- Experience in psychiatry, geriatrics, skilled nursing, assisted living, memory care, or community-based care preferred
- Passion for caring for older adults and supporting families through complex behavioral health challenges
- Strong clinical judgment and relationship-building skills
