JOB PURPOSE:
The Psychiatric Clinical Lead - Nurse Practitioner is an Advanced Practice Registered Nurse who plays a critical role in supporting integrated behavioral health care within a collaborative care model, focusing on mental health assessment and treatment for participants with psychiatric symptoms, cognitive deficits, and/or behavioral problems in a wide range of settings. This role provides psychiatric expertise to behavioral health care managers, primary care providers, and interdisciplinary teams, guiding treatment planning, monitoring patient progress, and ensuring timely care adjustments.
JOB RESPONSIBILITIES:
Performs a comprehensive psychiatric assessment of participants and develops treatment recommendations.
Provide regularly scheduled consultation to behavioral health care manager and IDT on a biweekly basis and as needed
Recommend any treatment adjustments through the case review process or in ad hoc consultations, including medication recommendations and other interventions, within 24 hours of consulting with a behavioral health care manager or other team members
Communicate the plan of care to the primary care providers in TeamCare and the community
Be available to provide occasional telephone consultation to medical providers for very complex patients or challenging patient situations. Respond to telephone calls from medical providers and behavioral health care managers within one business day. Respond to urgent telephone calls within two hours, if available.
Provides any of the following services to participants: advocacy, psycho-education, psychopharmacology, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions.
Prescribes pharmacologic agents based on a current knowledge of pharmacology and physiology.
Employs evidence based clinical practice guidelines to guide screening and diagnostic activities as available and as appropriate.
Evaluates the accuracy of the diagnosis and effectiveness of the intervention in relationship to the participant27s attainment of expected outcomes.
Identifies expected outcomes that:
Incorporate scientific evidence and are achievable through implementation of evidence-based practices
Incorporates cost and clinical effectiveness, participant satisfaction, and continuity and consistency among providers
Provides leadership in the coordination of multidisciplinary health care for integrated delivery of patient care services.
Provide in-service training for primary care-based providers and staff regarding recognition and treatment of behavioral health conditions in primary care.
Apply ethical and legal principles to the treatment of participants with mental health problems and psychiatric disorders.
Provides support and education to individuals, families, CLHC staff, and groups to promote knowledge, understanding and effective management of mental health problems and psychiatric disorders.
Maintains records in accordance with Behavioral Health and TeamCare standards and performs established workflow procedures
Maintains productivity sufficient to meet TeamCare standards.
Assumes responsibility for continued professional growth.
All other related duties as assigned by Supervisor.
Schedule: Monday - Friday | 8:00AM 2D 5:00PM
Weekly Hours: 40
Location: Telehealth [must be available to come in person for special cases].
QUALIFICATIONS:
Education:
Master27s degree in nursing from an accredited Nurse Practitioner program required.
Graduate-level specialization in Psychiatric/Mental Health Nursing required.
Doctor of Nursing Practice (DNP) preferred.
License/Registration:
Current, unrestricted New York State license as a Registered Professional Nurse (RN) and certification as a Nurse Practitioner (NP) with a specialty in Psychiatry/Psychiatric Mental Health required.
Current national board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) through an approved national certifying organization required.
Current DEA registration and New York State prescribing privileges required.
Must meet all applicable New York State requirements for prescribing controlled substances
Experience:
Minimum of one year27s experience as a Nurse Practitioner including prescribing preferred.
3-6 months27 experience in Geriatric Care is preferred.
Ability to work effectively as part of an interdisciplinary team.
Ability to work effectively with diverse populations and cultures.
Language: Bilingual preferred, but not required.
Additional:
Competency in the use of a personal/laptop computer primarily for clinical documentation.
Travel required 2D Ability to travel to other region/NY county as assigned.
Possession of a valid unrestricted driver27s licensed with a registered/insured vehicle highly preferred.
Physical Requirements
Individuals must be able to sustain specific physical requirements essential to the job. This includes, but is not limited to:
Standing 2D duration of up to 6 hours a day.
Sitting/Stationary positions 2D sedentary position for up to 6-8 hours a day for consecutive hours/periods of time.
Lifting/Push/Pull 2D up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely, using OSHA guidelines, etc.
Bending/Squatting 2D have to be able to bend or squat to care for patients safely, use medical supplies, etc.
Stairs/Steps/Walking/Climbing 2D must be able to maneuver stairs safely, climb up/down and walk to access work areas Position requires an individual to be able to travel, walk between sites/locations and work areas throughout the day.
Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (i.e., typing, use of medical supplies, equipment, etc.)
Sight/Visual Requirements 2D must be able to visually assess patients, read orders and type/write documentation, etc., accurately.
Audio Hearing and Motor Skills (language) Requirements 2D must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual.
Cognitive Ability 2D Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job.
