Qualifications

• Experience in managed care, value-based payment models, and population health management is essential.
• Excellent verbal and written communication skills for effective information delivery and presentations are crucial.
• Proven problem-solving abilities to navigate challenges effectively are necessary.
• Proficiency in MS Office applications including Word, Excel, PowerPoint, and Outlook is required.
• Willingness to travel up to 50% for provider meetings is essential.

Responsibilities

• Lead operational meetings with subject matter experts to implement new workflows and requirements.
• Conduct joint quarterly and monthly meetings to assess progress and identify performance improvement opportunities with leadership.
• Enhance provider experience through education, training, and effective communication of new initiatives.
• Manage critical relationships between providers and the health plan to ensure effective collaboration.
• Identify and propose enhancements to systems and processes for operational efficiencies.
• Drive performance engagement by presenting routine scorecards and health performance data to providers.
• Update dashboards and trend reports to keep stakeholders informed about provider performance.
