Full Job Description
Summary:

The Program Manager, Total Joint & Bariatric Services serves as the operational lead for the Total Joint and Bariatric Programs for the Western Region Hospitals. Strategically organizes the development, implementation, and organization of the episodic care program, including but not limited to value- based programs and associated Ambulatory Care Management services support. The Program Manager, Total Joint & Bariatric Services will coordinate the efforts between the surgeons, hospital administration and staff while tracking the progress of the programs and identifying areas for improvement. The Program Manager, Total Joint & Bariatric Services is the lead for The Joint Commission (TJC) Disease Specific Certification (DSC) for the Total Joint program and the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) and is responsible for all related activities to keep these designations current.

Responsibilities:

  • Creates and analyzes the operational plan for program strategies along with the Director of Care Coordination.
  • Analyzes data as it relates to the Orthopedic and Bariatric programs and communication to stakeholders.
  • Responsible for the longitudinal relationship and care to patients; including but not limited to pre-operative education, assistance with medical clearance, intraop and post-operative monitoring, discharge instructions and post hospital outreach.
  • Organizes all required meetings with surgeons and administrators regarding the programs.
  • Monitors disease specific compliance with the Joint Commission and MBSAQIP .
  • Leads staff education on new and revised standards and elements of performance, providing staff support for the readiness of the multidisciplinary staff.
  • Coordinates the overall assessment, design, and implementation of the Joint Commission and MBSAQIP action plans, tracking quality measures, analyzing progress and implementing changes to meet the standards of care. This is accomplished in close collaboration with key stakeholders for functional areas throughout the organization and the ability to manage projects and the PI plan.
  • Obtains, analyzes, and stratefies all data necessary for the performance measures and enters data in a timely manner in specified program sites.
  • Develops positive relationships with physician and staff.
  • Performs other duties as assigned.


Qualifications/Requirements:

Experience:

Minimum of 2 years' experience in an acute care clinical setting required.

2 plus years of experience working with surgeons and other physicians' specialties preferred.

2 plus years of experience providing care to total joint patients preferred.

Prior experience of patient and staff education preferred.

Education: Graduation from an accredited school of Nursing required; bachelor's degree in nursing preferred.

Licenses / Certifications: Current NYS Nursing license required.
