Job SummaryPerforms a wide range of professional tasks to administer and facilitate the management of various programs in the Home & Community Services Division of the Department of Aging.
The hiring salary for this position is $62,524 annually; full salary range for this position is $62,524 - $107,577 annually.
Essential Functions - Provides coordination of services for the above listed programs for consumers and families receiving home and community-based services. Assures consumers receive needed and appropriate services and coordinates the provision of services by the Department and other partnering agencies;
- Provides support services for the National Family Caregiver Support Program for consumers and families receiving home and community-based services under this program;
- Provides coordination of respite care for consumers and families receiving home and community-based services under this program;
- Provides leadership and coordination of the S.O.A.R. Committee;
- Oversees Case Management Services for all S.O.A.R. Program recipients;
- Speaks with consumers and family members to relay information to Caseworker;
- Conducts assessments and provides home visits and deliveries; to assist Caseworker
- Conducts quarterly visits, which include defined tasks; to assist Caseworker
- Handles consumer purchases as requested by the Caseworker;
- Writes detailed case notes as appropriate;
- Supports caregiver training/education;
- Fiscal reconciliation of all consumer and program expenditures;
- Attends all mandatory State Meetings and Training;
- Supports the HCBS Division, the Department of Aging & Humans Services and County Government in a positive manner;
- Performs other duties as assigned.
Required Knowledge, Skills, and Abilities - Ability to gain thorough knowledge of St. Mary's County Government policies and procedures;
- Ability to effectively communicate with other staff and especially members of the public in a courteous and caring manner;
- Ability to gain knowledge of department practices and procedures and of the operations, procedures, and principles of assigned program;
- Ability to gain knowledge of department's fiscal practices and procedures and of the fiscal operations, procedures, and principles of assigned program(s);
- Ability to gain knowledge of Federal, State and Local regulations regarding assigned programs;
- Knowledge of County and Federal fiscal management;
- Ability to effectively communicate with other staff members; ability to coordinate, advise, and work with other professionals;
- Ability to prioritize and multitask;
- Ability to review and analyze existing information and make informed and sound decisions;
- Ability to use available resources to research information;
- Ability to keep accurate records; and
- Ability to operate relevant computer systems, including hardware and software.
Education and Experience - Bachelor's degree in a human services field, including psychology, social work, sociology, nursing, counseling, sociology or a related field preferred.
- A minimum of three years' experience pertaining to case management for individuals with complex medical and/or behavioral health needs, older adults and/or adults living with disabilities.
- A minimum of one year's experience with administrative fiscal practices
Physical and Environmental Conditions:Work requires no unusual demand for physical effort. Some consumer supplies, and medical equipment for storage/delivery purposes, may require minor amounts of lifting/transfers.
Work environment involves everyday risks or discomforts associated with the position, which require standard safety precautions typical of such places as offices, meeting rooms, consumer homes and long-term care facilities. This position also requires in-home visits. Use of safe workplace practices with office equipment, and/or avoidance of trips and falls are necessary.
