Job Summary
Performs a wide range of professional tasks to administer and facilitate the management of various programs in the Home & Community Services Division of the Department of Aging.

The hiring salary for this position is $62,524 annually; full salary range for this position is $62,524 - $107,577 annually.

Essential Functions

  1. Provides coordination of services for the above listed programs for consumers and families receiving home and community-based services. Assures consumers receive needed and appropriate services and coordinates the provision of services by the Department and other partnering agencies;
  2. Provides support services for the National Family Caregiver Support Program for consumers and families receiving home and community-based services under this program;
  3. Provides coordination of respite care for consumers and families receiving home and community-based services under this program;
  4. Provides leadership and coordination of the S.O.A.R. Committee;
  5. Oversees Case Management Services for all S.O.A.R. Program recipients;
  6. Speaks with consumers and family members to relay information to Caseworker;
  7. Conducts assessments and provides home visits and deliveries; to assist Caseworker
  8. Conducts quarterly visits, which include defined tasks; to assist Caseworker
  9. Handles consumer purchases as requested by the Caseworker;
  10. Writes detailed case notes as appropriate;
  11. Supports caregiver training/education;
  12. Fiscal reconciliation of all consumer and program expenditures;
  13. Attends all mandatory State Meetings and Training;
  14. Supports the HCBS Division, the Department of Aging & Humans Services and County Government in a positive manner;
  15. Performs other duties as assigned.

Required Knowledge, Skills, and Abilities

  1. Ability to gain thorough knowledge of St. Mary's County Government policies and procedures;
  2. Ability to effectively communicate with other staff and especially members of the public in a courteous and caring manner;
  3. Ability to gain knowledge of department practices and procedures and of the operations, procedures, and principles of assigned program;
  4. Ability to gain knowledge of department's fiscal practices and procedures and of the fiscal operations, procedures, and principles of assigned program(s);
  5. Ability to gain knowledge of Federal, State and Local regulations regarding assigned programs;
  6. Knowledge of County and Federal fiscal management;
  7. Ability to effectively communicate with other staff members; ability to coordinate, advise, and work with other professionals;
  8. Ability to prioritize and multitask;
  9. Ability to review and analyze existing information and make informed and sound decisions;
  10. Ability to use available resources to research information;
  11. Ability to keep accurate records; and
  12. Ability to operate relevant computer systems, including hardware and software.

Education and Experience

  1. Bachelor's degree in a human services field, including psychology, social work, sociology, nursing, counseling, sociology or a related field preferred.
  2. A minimum of three years' experience pertaining to case management for individuals with complex medical and/or behavioral health needs, older adults and/or adults living with disabilities.
  3. A minimum of one year's experience with administrative fiscal practices

Physical and Environmental Conditions:

Work requires no unusual demand for physical effort. Some consumer supplies, and medical equipment for storage/delivery purposes, may require minor amounts of lifting/transfers.

Work environment involves everyday risks or discomforts associated with the position, which require standard safety precautions typical of such places as offices, meeting rooms, consumer homes and long-term care facilities. This position also requires in-home visits. Use of safe workplace practices with office equipment, and/or avoidance of trips and falls are necessary.

  1. Must successfully pass background investigation with favorable results;
  2. Must pass a pre-employment drug test.
