DescriptionJanian Medical Care is seeking dynamic, collaborative, and mission-driven Primary Care Providers who are committed to delivering high-quality, person-centered care to individuals with lived experience of homelessness. Duties and Responsibilities- Provide comprehensive, high-quality primary care to adults with lived experience of homelessness in community-based settings, including permanent supportive housing, transitional shelters, safe havens, drop-in centers, and through Street Medicine.
- Deliver person-centered, trauma-informed, and evidence-based care to medically complex and vulnerable populations.
- Collaborate closely with interdisciplinary teams, including psychiatry, behavioral health clinicians, social workers, case managers, and street outreach staff, to coordinate care and address medical, behavioral health, and social needs.
- Participate in integrated care models through co-located clinical services and regular interdisciplinary collaboration.
- Maintain accurate, timely clinical documentation, utilizing protected administrative time to support quality care and compliance.
- Manage a balanced and sustainable patient caseload with a focus on quality outcomes rather than volume.
- Participate in ongoing professional development activities, including weekly protected educational time and an accredited Grand Rounds series.
- Engage in program development and continuous quality improvement initiatives to strengthen service delivery and client outcomes.

Requirements- Board Certified or Board Eligible NP licensed to practice medicine in New York State, with training in family medicine, internal medicine, or other primary care specialty
- Good communication and collaboration skills
- Commitment to person-centered practices
- Commitment to servant leadership principles
- Proficient in the use of electronic medical records (eClinicalWorks preferred)
- Proficient in the use of Microsoft Office, including Outlook
- Experience working with people living with mental illness and/or with homeless or justice-involved people preferred
- Bilingual Spanish speaker preferred
