Primary Care Provider (NP/PA/MD/DO) - Mobile Primary Care Base Salary: $150,000 + Bonus Eligibility Billet Health is seeking an experienced and entrepreneurial Primary Care Provider to join our rapidly growing Mobile Primary Care division serving the Phoenix metropolitan area. This is a full-time, Monday-Friday (8:00 AM-5:00 PM) opportunity for a provider who is passionate about delivering high-quality, patient-centered care directly in community and residential settings. Practice Settings Providers will deliver care in: - Assisted Living Facilities - Skilled Nursing Facilities - Memory Care Communities - Group Homes - Private Residences - Retirement Communities Responsibilities - Conduct comprehensive patient evaluations and physical exams - Diagnose and manage acute and chronic illnesses - Develop individualized treatment plans - Prescribe medications and manage ongoing therapies - Review diagnostics and coordinate follow-up care - Perform minor procedures and wound care (preferred) - Maintain timely and accurate documentation - Collaborate with interdisciplinary healthcare teams Qualifications - Active Arizona license as an NP, PA, MD, or DO - DEA license with prescribing authority - Board certification where applicable - Minimum of 1-2 years of clinical experience - Family Medicine experience preferred - Wound care experience strongly preferred - Strong clinical judgment and ability to practice independently Compensation & Benefits - Base Salary: $150,000 annually - Eligible for performance-based bonus opportunities - Medical, dental, and vision insurance - 401(k) - Paid Time Off - Malpractice coverage - Company-provided equipment and supplies - Dedicated back-office and Medical Assistant support Territory Phoenix Metro Area, including Gilbert, Chandler, Mesa, Tempe, Scottsdale, Phoenix, Glendale, Peoria, Sun City, and Surprise.
