Primary Care Physician to join our team supporting the Hyannis Community Based Outpatient Clinic in Hyannis, Massachusetts . This is an outstanding opportunity to provide comprehensive, patient-centered outpatient care to Veterans while working alongside a collaborative interdisciplinary healthcare team committed to clinical excellence.

About the Opportunity

As a Primary Care Physician, you will provide comprehensive outpatient primary care services to eligible Veterans through the Veterans Health Administration (VHA). Working within the Patient Aligned Care Team (PACT) model, you will evaluate, diagnose, treat, and manage a wide range of acute and chronic medical conditions while promoting preventive healthcare and wellness. This role offers the opportunity to make a lasting impact on the health and well-being of those who have served our country.

Schedule: Mon-Fri, 8:00am-4:30pm

What You'll Do
  • Provide comprehensive outpatient primary care, including preventive, acute, and chronic disease management
  • Perform patient assessments, medical histories, physical examinations, and develop individualized treatment plans
  • Order, interpret, and communicate laboratory, imaging, and diagnostic test results
  • Prescribe medications and monitor treatment effectiveness in accordance with VA guidelines
  • Coordinate referrals and collaborate with specialists and multidisciplinary healthcare teams
  • Promote preventive health through screenings, immunizations, wellness counseling, and patient education
  • Maintain accurate and timely documentation within the VA electronic health record
  • Participate in quality improvement initiatives and maintain compliance with VA policies, patient safety standards, and privacy regulations
  • Foster effective communication with Veterans, their families, and the healthcare team to ensure continuity of care
  • Complete required VA credentialing, privileging, mandatory training, and continuing medical education requirements

Required Qualifications
  • Active, unrestricted Physician license
  • Board Certified or Board Eligible in Internal or Family Medicine
  • Current DEA registration
  • National Provider Identifier (NPI)
  • Current American Heart Association (AHA) Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications
  • Successfully complete the AMI credentialing process
