Full Job Description
We are seeking a primary care provider (NP or PA) to work 30-40 hours per week in Adult Medicine. The Adult Medicine Clinic is a vibrant primary care setting with a close-knit team of medical providers, nurses, and medical assistants, supported by behavioral health clinicians, dietitians, and health educators, as well as on-site pharmacy, acupuncture, optometry, and dental services. This position reports to the Chief Medical Officer and is an exempt role with a salary range of $151,376 to $166,015 (1 FTE) Based on experience.
Responsibilities
• Provide comprehensive primary care, including preventive care, chronic disease management, and acute care.
• Conduct medical assessments, develop treatment plans, and coordinate referrals.
• Collaborate with a multidisciplinary team, including nurses, behavioral health providers, and care managers.
• Maintain accurate documentation in the Electronic Health Record (Epic preferred) to ensure continuity of care.
• Participate in quality improvement initiatives to enhance patient outcomes.
• Advocate for health equity and culturally responsive care.
MINIMUM QUALIFICATIONS
• Nurse Practitioner or Physician Assistant certified by a Family, Adult/Geriatric Program
• Valid California and DEA License
• Fluent in Spanish.
• Passion for community health and serving diverse populations.
