Advanced Practice Provider - Primary CareIn this Role: As a Primary Care Advance Practice Provider, you will provide comprehensive, longitudinal patient-centered care in an outpatient setting while collaborating with an experienced multidisciplinary team.
- Perform comprehensive patient evaluations and physical examinations
- Diagnose and manage common acute illness and chronic conditions, including hypertension, diabetes, asthma, and other common primary care conditions
- Develop, implement and evaluate individualized care plans
- Order, interpret, and follow up on labs, imaging, and other diagnostic tests
- Prescribe medications and treatments consistent with current clinical guidelines
- Provide age-appropriate care, including adult and geriatric care (and pediatrics, if capable)
- Coordinate in transitions of care, including hospital follow-ups and post discharge management
- Collaborate with Bayview specialists through an integrated internal referral network
- ~ 18-20 patients/day
- Work and collaborate with other Physicians and APPs within the practice

What we offer:- Hiring Range: $115,000-$145,000
- Actual pay depends on specialty, experience, schedule, and job duties
