Short Description

The Registered RN is a professional care-giver who, as a member of a multidisciplinary health care team, assumes responsibility and accountability for a group of patients for a designated time frame and who provides care to these patients utilizing the nursing process including the therapeutic use of self.

 

Position SummaryThe RN Triage Nurse serves as the clinical triage and care coordination resource for the Population Health at Home program. This role is responsible for assessing patient concerns, reviewing clinical information, coordinating care, managing test results, providing patient education, and escalating urgent issues to ensure safe, timely, and effective patient care.Essential Duties & Responsibilities•    Provide telephonic and electronic clinical triage, assessing patient symptoms and determining the appropriate level of care.•    Utilize evidence-based protocols and clinical judgment to manage urgent concerns, escalate high-risk situations, and facilitate timely interventions.•    Review and communicate laboratory, diagnostic, and provider results, ensuring appropriate follow-up and patient understanding.•    Coordinate care across providers and interdisciplinary teams to support seamless patient management and transitions of care.•    Identify and engage eligible patients for Population Health at Home programs, promoting preventive care, chronic disease management, and improved health outcomes.•    Serve as a clinical resource for complex patient issues, ensuring concerns are resolved efficiently and safely.•    Deliver patient education and self-management support to enhance engagement and adherence to care plans.•    Maintain accurate and timely documentation in the electronic medical record while adhering to regulatory, quality, and organizational standards.
