About the Role

We are seeking an experienced healthcare business development professional to drive census growth for skilled nursing facilities serving the Rockford market. This role is responsible for developing and maintaining strong referral relationships with hospital case managers, discharge planners, physicians, and healthcare leaders while ensuring a seamless transition of patients into our facilities.

The ideal candidate is well-connected within the Rockford healthcare community and has experience working with hospitals such as OSF HealthCare, Mercyhealth, and UW Health SwedishAmerican.
Key Responsibilities
  • Develop and execute strategies to increase admissions and census for two skilled nursing facilities.
  • Build and maintain trusted relationships with hospital case managers, discharge planners, social workers, physicians, and referral partners.
  • Serve as the primary liaison with local hospitals, including OSF HealthCare, Mercyhealth, and UW Health SwedishAmerican.
  • Conduct regular in-person hospital visits and maintain a consistent presence with referral sources.
  • Identify new referral opportunities and strengthen existing partnerships.
  • Collaborate with facility leadership and admissions teams to ensure timely, efficient admissions.
  • Monitor referral activity, conversion rates, and census trends to achieve growth goals.
  • Represent the organization at networking events, healthcare meetings, and community outreach opportunities.
  • Maintain a thorough understanding of payer sources, post-acute care services, and market dynamics.
Qualifications
  • Bachelor's degree preferred.
  • Minimum of 3 years of healthcare business development, physician relations, hospital liaison, or admissions experience.
  • Existing relationships within the Rockford healthcare market strongly preferred.
  • Knowledge of skilled nursing, post-acute care, Medicare, managed care, and hospital discharge planning.
  • Exceptional communication, relationship-building, and negotiation skills.
  • Self-motivated with a proven track record of growing census and developing referral networks.
  • Ability to travel throughout the Rockford area on a daily basis.
