Holds a valid and unrestricted PA license from the Arizona Regulatory Board.
Possesses active Board Certification from the NCCPA.
Has current DEA registration to prescribe controlled substances.
Underwent N95 respirator fit testing and holds the required documentation.
Completed the necessary Delegation Agreement to practice in a supervisory role.
Minimum of two years of relevant work experience.
