Job DescriptionPosition Overview We are seeking a compassionate and motivated Physical Therapist to join a dynamic rehabilitation team. In this role, you'll evaluate patients, develop individualized treatment plans, and help individuals regain mobility, improve function, and achieve their rehabilitation goals. Responsibilities
- Evaluate patients and develop individualized treatment plans based on clinical findings and physician referrals.
- Implement evidence-based therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence.
- Monitor patient progress and modify treatment plans as needed to maximize outcomes.
- Educate patients and family members on treatment plans, home exercise programs, injury prevention, and wellness strategies.
- Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory standards.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team.
- Ensure a safe treatment environment by following infection control and safety protocols.
- Participate in interdisciplinary care planning and discharge planning.
- Maintain compliance with all state, federal, and organizational policies and procedures.
- Perform additional duties as assigned to support the rehabilitation department.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the applicable state or eligibility to obtain licensure.
- Current CPR/BLS certification (if required).
- Strong clinical assessment and treatment planning skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
