LTC Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team at GlenRidge/Courtyard in Medford, MA. This role offers the opportunity to provide individualized therapy services within a skilled nursing setting focused on improving resident mobility, strength, safety, and overall functional performance.
Position Responsibilities:- Perform physical therapy evaluations and establish individualized treatment goals
- Provide therapy interventions focused on gait training, balance, strengthening, endurance, transfers, and mobility training
- Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care plans
- Monitor resident progress and modify treatment approaches as clinically appropriate
- Maintain accurate documentation and treatment records in compliance with facility standards

Qualifications:- Active Physical Therapist license required
- Strong communication and clinical reasoning skills
