Physical Therapist (PT) to join our exceptional
in-house rehabilitation team.
Our short-term rehabilitation and skilled nursing programs provide a dynamic, patient-centered environment where clinicians work collaboratively to achieve outstanding outcomes and help patients return to their highest level of independence.
Work alongside experienced
Physical, Occupational, and Speech Therapists, nursing professionals, and interdisciplinary care teams. Our collaborative approach ensures every patient receives comprehensive, individualized care.

As a Physical Therapist, you'll help patients regain
strength, mobility, balance, and confidence while developing individualized rehabilitation programs focused on maximizing independence and quality of life.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and develop individualized treatment plans.
- Provide evidence-based interventions to improve mobility, strength, balance, coordination, and functional independence.
- Monitor patient progress and modify treatment plans based on clinical findings and patient response.
- Supervise Physical Therapist Assistants in accordance with California State Practice Act requirements.
- Collaborate with physicians, nursing staff, therapy professionals, and interdisciplinary care teams.
- Participate in discharge planning and help patients transition safely to their next level of care.
- Recommend appropriate durable medical equipment (DME) and assist patients and families with safe mobility strategies.
- Educate patients and caregivers on home exercise programs, safety, and functional mobility.
- Maintain accurate, timely, and compliant clinical documentation.
Qualifications
- Current California Physical Therapist (PT) license required
- Strong clinical, communication, and interpersonal skills
- Commitment to compassionate, patient-centered care
- Ability to work effectively in a collaborative interdisciplinary environment
