LTC Therapy is hiring a Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This position offers the opportunity to work with residents recovering from orthopedic procedures, neurological conditions, illness, and mobility decline while supporting functional recovery and independence.
Position Responsibilities:- Complete physical therapy evaluations and establish individualized treatment plans
- Provide therapy focused on strength, balance, gait training, mobility, endurance, and pain management
- Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care goals
- Monitor resident progress and adjust treatment approaches as needed
- Maintain accurate and compliant documentation

Qualifications:- Active PT license required
- Strong communication and clinical decision-making skills
- Skilled nursing experience preferred but not required
