Your Role and Impact:- Lead with Expertise: Upon referral, conduct comprehensive evaluations of patients' physical function-including strength, mobility, motor control, joint health, pain levels, balance, posture, and cardiopulmonary performance. You'll assess vitals, endurance, gait, and daily living capabilities with precision and compassion.
- Create Impactful Care Plans: Design personalized, patient-centered treatment plans that promote optimal movement, safety, and long-term wellness. Your clinical insight will guide each step of the recovery journey.
- Deliver Innovative Therapy: Implement and delegate care using a wide range of therapeutic interventions-from heat, cold, and light therapy to mobilization and targeted exercise. You'll use your clinical judgment to tailor treatments that truly work.
- Collaborate for Continuity: Work closely with a multidisciplinary team-including Rehab Therapists, Nurses, and Social Workers-to ensure seamless care coordination. You'll also play a key role in discharge planning and connecting patients with community resources to support their ongoing recovery.
Qualifications:- Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school; Bachelor's, Master's or Doctoral degree in Physical Therapy
- CT active or temporary license
