In this role, you will provide home safety evaluations, assess the patient's current and prior level of functioning, develop an individualized treatment plan, and guide the patient towards greater independence and improved health outcomes. Treat a diverse patient population including post-operative joint replacements, neurological, cardiac, respiratory, renal, and oncology patients.
