**$15,000 Sign On Bonus (conditions apply)** ResponsibilitiesPhysical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
The PT will: • Evaluate the patient's need for physical therapy and develop an appropriate treatment plan. • Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
- Communicate with all members of the health care team.
- Document evaluations, treatment goals, and objectives.
- Teach patients and family members the rationale for treatments and procedures.

Qualifications REQUIRED QUALIFICATIONS - Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school in Physical Therapy
PREFERRED QUALIFICATIONS - One year of experience as a PT in a home health care setting is preferred
Required Licensure/Certifications- Unencumbered PT license in VT and NH or compact. - Driver's license, minimum requirements on car insurance, and a clean driving record

- Pay Range: $81,640.00/Yr. - $126,547.20/Yr. (Based on 40 hours per week, otherwise pro rata)
