Full Job Description
Under the supervision of the Assistant Director of Pharmacy, the Staff Pharmacist performs all duties necessary to provide effective daily operation of all pharmacy departmental services and programs. Schedule is varied and responsibilities include: Compounding, labeling, and dispensing of medications and IV admixtures; Provide therapeutic drug monitoring by profiling physician orders; Computer order entry of physician orders to build medication profiles; Facilitate IV / oral medication preparation and dispensing; Supervise technicians in their daily duties; Provide pharmacokinetic drug dosing when requested by a physician.

Essential Functions:
  1. Prepares and dispenses medication orders per physician request according to established policy, procedure, and protocol on a regular basis.
  2. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies on a daily basis.
  3. Interprets medication orders and transcribes to computerized patient medication profiles accurately.
  4. Compounds and dispenses pharmaceuticals, including sterile, and parental nutrition products accurately on a regular basis.
  5. Monitors drug therapy regimens for contraindications, drug Â- drug interactions, drug Â- food interactions, allergies, and appropriateness of drug and dose; Detects and reports suspected adverse drug reactions. On a regular basis.
  6. Provides clinical consultation, clarification, and drug information as needed to practitioners and/or patients.
  7. Participates in the development and presentation of orientation, education, and training programs to the pharmacy, medical, nursing, and other staff members on an as needed basis.
  8. Contributes to the quality and effective operation of the department.
  9. Documents all clinical interventions promptly and accurately on a regular basis.
  10. Provides excellent customer service to patients and family members.
  11. Assist other pharmacy staff members as needed.
  12. All other duties assigned


Requirements

Educational / Experience Requirements:
  • Formal Education: Bachelor's Degree
  • Experience: 6 months
  • License, Registration, and / or Certification Requirement: Yes
  • Bachelor of Science Pharmacy Degree required
  • Graduate of an ACPE accredited School of Pharmacy required
  • Hospital experience, preferred
  • Current license to practice pharmacy in the state of practice required

Qualifications / Skills:
  • Strong written and verbal communication skills
  • Strong mathematical and computer literacy skills

Work Environment:
  • Occupational Risks: Some portions (less than 50%) of daily assignments involve occupational risk, such as cuts, burns, exposure to toxic chemicals, injuries from falls, or back injury sustained with assisting in moving, lifting, or positioning patients, equipment, or materials.
  • Safety Equipment: Job does not require the use of safety equipment, such as HEPA mask, for infection prevention.
  • Bloodborne Pathogens Exposure: OSHA Category I - Exposure not anticipated. Work does not involve exposure to blood, body fluids, or tissue, and does not require performing unplanned exposure tasks.
  • Ergonomic Risks: Some portions (less than 50%) of daily assignments involve ergonomic risk, such as regular repetitive tasks, forceful or prolonged exertions of the hands, vibration, cold temperatures, heavy lifting, pulling, pushing, or carrying of heavy objects, poor body mechanics, restrictive workstations, or awkward postures. Lifting: Job requires lifting up to 30 pounds.
  • Climbing Stairs: Job does not require climbing stairs.
  • Manual Skills: Significant portions (more than 50%) of daily assignments involve application of manual skills requiring motor coordination in combination with finger dexterity, e.g., typing or machine operations.
  • Physical Effort: Some portions (less than 50%) of daily assignments involve prolonged standing, transporting patients, material or equipment, or lifting, moving, or carrying heavy (over 30 pounds) materials for intermittent periods throughout the day.
  • Work Schedule: Work may require evening, night, holiday, or weekend assignments on occasion.
  • Physical Environment: Generally pleasant working conditions.
  • Volatility of Workload: The workload required to perform this job requires infrequent changes, but when changes occur they tend to be minor.
  • Travel: Some portions (less than 50%) of daily assignments require travel to client/resident/patient locations or other work sites, via car or public transportation.
