Qualifications

Full Job Description
He/She is responsible for providing comprehensive, patient-centered, clinical pharmacy services for assigned areas. They will proactively influence medication use and make recommendations concerning the patient's medication profile. The clinical pharmacist will assess medication orders for appropriateness of therapy taking into account patient-specific factors, and evaluate medication regimens to assure positive outcomes including relevant lab results, tests and/or procedures. He/She will oversee drug distribution and preparation assuring safety and timeliness. They will initiate and participate in the collaborative management of patients with other members of the healthcare team, including medication reconciliation, core measures compliance, anticoagulation management, target drug programs, medication usage guidelines, patient counseling, discharge counseling and performance improvement initiatives. The clinical pharmacist will participate in continuous readiness efforts to maintain Joint Commission Patient safety Goals and Medication Management Standards. They will precept pharmacy and technician students in collaboration with the pharmacy management team. He/She will actively participate in developing and presenting educational topics, and staff development programs when requested
