Qualifications

• Doctor of Pharmacy (PharmD) degree required.
• Valid Pennsylvania Pharmacist License, including Injectable License for vaccine administration.
• Experience in a Specialty Pharmacy preferred.
• Familiarity with Outcome Platforms necessary.
• Medication Therapy Management (MTM) certification preferred.
• Board Certified Ambulatory Care Pharmacist (BCACP) preferred.
• Current CPR certification required.
• Spanish language skills advantageous.
• Collaborative mindset with healthcare professionals expected.
• Excellent communication and interpersonal skills required.

Responsibilities

• Provide exceptional pharmaceutical care to patients.
• Collaborate with healthcare teams to ensure medication safety and effectiveness.
• Manage medication therapy and conduct patient assessments.
• Educate patients on medication use and health management.
• Administer vaccines as authorized and educate patients on immunization.
• Collect and analyze patient data to optimize drug therapies.
• Stay current with new medications and practices in the field.
