Unit Summary
The Pharmacist promotes evidence-based/cost effective drug therapy, provides drug information, participates in quality improvement activities and promotes clinically relevant research. Pharmacists will be at times required to mentor Pharmacy residents, students, and interns. Being an integral member of the Pharmacy team, the Pharmacist has a comprehensive understanding of the drug distribution system and participates in processes that ensure safe medication practices (e.g. computerized order entry, medication reconciliation). In addition, Pharmacists are required to participate in Pharmacy related activities (e.g. committees, review of order sets) and duties as assigned.
Position Summary
The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes. This work may be performed on individual clinical units, or may be across multiple units to support the medication system needs of the site.

Days, Evenings, Nights, Weekends, Statutory Holidays, Over-Night On-Call
Qualifications
1. Licensed or eligible for license by the Ontario College of Pharmacists Part A of the Registry
2. Baccalaureate of Science in Pharmacy or Doctor of Pharmacy Degree
3. Demonstrated computer skills including pharmacy systems, word processing, spreadsheet, and e-mail systems
4. Demonstrated ability to function as an independent practitioner
5. Demonstrated innovation and project leadership skills
6. Demonstrated commitment to continuous learning
7. Demonstrated excellence in independent practice, teamwork, change management, teaching, problem-solving and communication skills
8. Hospital Pharmacy Residency preferred
