Qualifications

• Bachelor's degree from an accredited School of Pharmacy or Pharmacy Doctorate
• Current license required; dual license preferred
• One-year experience in acute, long-term care or home health preferred
• Consultant or Clinical Pharmacist experience preferred
• IV clinical experience preferred
• Must be able to work weekends or outside of normal business hours if needed
• Ability to lift patient charts repetitively
• Ability to stand for long periods of time
• Good customer service skills
• Understanding of state and federal regulations for long-term care
• Knowledge of formulary management

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions, and stability; recommend therapy changes as needed
• Review drug regimens and clinical indicators of residents at least monthly; report irregularities with written findings
• Document all communications with healthcare team members, patients, and their families
• Maintain knowledge of pharmacy laws and regulations related to long-term care
• Cooperate effectively as a team member
• Stay updated on pharmaceutical care practices for long-term care residents
