ROLE SUMMARY:

We are seeking a highly skilled and motivated South Carolina licensed pediatric pharmacist to join our team in providing comprehensive medication management services for payors in a completely remote working environment. This remote position involves working closely with healthcare providers, insurance professionals, and pediatric caregivers/patients to optimize medication therapy outcomes. The ideal candidate will possess a strong background in pediatric pharmacy, excellent communication skills, and the ability to work independently.

KEY RESPONSIBILITIES:

Medication Review and Management:

  • Conduct thorough medication reviews for pediatric patients, ensuring appropriateness, safety, and efficacy.
  • Collaborate with healthcare providers to develop and implement comprehensive medication management plans for pediatric patients.

Remote Consultations:

  • Provide remote consultations to caregivers to address medication-related concerns.
  • Offer recommendations for medication adjustments, therapeutic alternatives, adherence strategies, etc.

Patient Education:

  • Educate pediatric patients and their caregivers on proper medication use, potential side effects, and the importance of adherence.
  • Provide guidance on lifestyle modifications and other non-pharmacological interventions to optimize patient outcomes.

Collaborative Care:

  • Work collaboratively with interdisciplinary healthcare teams to ensure seamless communication and coordination of care.
  • Participate in case conferences and team meetings to discuss complex cases and develop integrated care plans.

Documentation:

  • Maintain accurate, timely and up-to-date documentation of medication management interventions.
  • Ensure compliance with regulatory requirements and organizational policies.

Continuous Professional Development:

  • Stay abreast of the latest developments in pediatric pharmacotherapy through ongoing education and training.
  • Contribute to the development of evidence-based guidelines and protocols for pediatric medication management.

Quality Improvement Initiatives:

  • Participate in quality improvement initiatives to enhance the overall effectiveness of medication management services.
  • Collect and analyze data to evaluate the impact of interventions on patient outcomes.
  • Other duties as assigned.

QUALIFICATIONS:

  • Doctor of Pharmacy (PharmD) from an accredited program; active pharmacy license required. Additional state licensure, upon request.
  • Board certification in Pediatric Pharmacy (BCPPS), preferred.
  • Minimum 4 years of pediatric pharmacy experience with strong clinical knowledge across disease states.
  • Excellent communication and interpersonal skills; able to convey complex information clearly in virtual settings.
  • Proficient in telehealth platforms, EHR systems, and online collaboration tools.
  • Strong organizational and time-management skills with attention to detail and remote documentation accuracy.
  • Proven ability to work independently, manage caseloads, and meet productivity goals.
  • Skilled in influencing and engaging patients and providers through effective, empathetic communication.
