Overview

OBJECTIVE

The PACE Clinical Consultant Pharmacist is a member of the Grane Rx team that works as a Clinical Consultant Pharmacist for a PACE organization. The PACE Clinical Consultant Pharmacist will provide recommendations and guidance to the center’s interdisciplinary teams to improve patient and center clinical outcomes associated with medication use in addition to proactively identifying and recognizing industry trends and issues that have the ability to clinically and/or financially affect PACE customers. The PACE Clinical Consultant Pharmacist promotes Grane Rx’s comprehensive pharmacy services in a proactive positive manner while articulating the needs of our customers to the pharmacy team.

 

KEY RELATIONSHIPS

The PACE Clinical Consultant Pharmacist works extensively on a daily basis with the PACE organizations serviced by Grane Rx and their clinical teams to provide exceptional clinical pharmacist services, medication regimen reviews, and clinical programs designed to improve participant and organization clinical outcomes associated with medication use.

Responsibilities

DUTIES AND RESPONSIBILITIES

• Provides clinical consultant pharmacist services to PACE Primary Care Providers and other clinic staff

• Performs drug regimen reviews for all program participants. Strong clinical focus on geriatric pharmacy issues to improve quality of life and reduce the risk for adverse effects

• Performs medication storage audits and EMR audits related to medication processes

• Educates staff regarding medication related issues as needed

• Collaborates with Medical Director and center management team to develop and initiate medication related initiatives and projects as needed

• Visits each PACE center on a scheduled basis to promote increased facility interaction, collaboration and to foster improved clinical and customer service outcomes

• Responsible for maintaining optimal customer service levels through establishing working team relationships

• Participates in the Grane Rx consulting team departmental meetings

PHYSICAL, MENTAL AND SENSORY REQUIREMENTS

• Ability to read, write, speak and comprehend English

• Able to carry 15 to 20 pounds

• Uses good judgment and independent decision-making

• Travel to customer locations to perform job functions; overnight travel when necessary

• Excellent verbal and written communication skills

• Has ability to be mobile in performing job duties

• Computer literate

Qualifications

WORK EXPERIENCE REQUIREMENTS

 

Three or more years of consultant pharmacist experience in PACE, long term care, or another geriatric related field.

 

Fluency in both English and Korean (written and spoken)

 

EDUCATIONAL REQUIREMENTS

 

Graduate of an ACPE accredited School of Pharmacy with a BS in Pharmacy or Doctor of Pharmacy degree required

 

PGY1 Residency preferred

 

Advanced Pharmacist Practitioner license strongly preferred

 

Certified Geriatric Pharmacist preferred or willingness to obtain certification upon hire

 

Registered pharmacist in the state of California and states where services will be provided

 

 

SKILLS, LICENSE, CERTIFICATION AND/OR SPECIAL TRAINING REQUIREMENTS

· Extensive experience in long term care pharmacy

· Experience with the LIFE/PACE program preferred but not required

· Extensive medication management experience

· Strong data analytics skills

· Ability to work with teams and be a team leader

· Ability to work independently and be self-motivated

· Ability to prioritize assigned tasks

· Attention to detail and completion of all assigned tasks

· Ability to work in a fast paced, challenging, clinical and technical environment

· Reliable and consistent in timeliness, attendance and quality of work.

· Advanced proficiency with Microsoft Office products
