Responsibilities

  • Coordinate Patient Performance Data (PPD) collection with the team.
  • Ensure medical records documentation for Quality Indicators (QIs) is complete.
  • Complete admissions and discharges of IRF-PAIs within specified timeframes.
  • Extract information from medical records and summarize it on the IRF-PAI.
  • Enter data into CMS-approved software within required timeframes.
  • Maintain and verify accuracy of the IRF-PAI log based on patient roster.

Facility: UC Health - Irvine Rehabilitation Hospital

Schedule: Full Time

Join us in opening UC Health - Irvine Rehabilitation Hospital, a brand-new 52 bed inpatient rehabilitation facility (IRF) opening its doors in summer of 2026. 

How you’ll contribute 

A Outcomes Manager who excels in this role:

  • Coordinate PPD data collection with the interdisciplinary team.

  • Ensure supportive documentation for QIs is complete in the medical record.

  • Complete admission and discharge IRF-PAIs within CMS-designated timeframes.

  • Abstract required information from medical records and summarize on the IRF-PAI.

  • Enter data into the CMS-accepted software/program within designated timeframes.

  • Maintain and update the IRF-PAI log; verify accuracy against the patient location roster.

What we’re looking for:

Salary Range: $110,000-$115,000
