Job Description:
TheOncologyNurseNavigatoris responsible fornavigating and optimizing the experience of Prostate Cancer patients as they move through the continuum of cancer care, from screening and detection to diagnosis, treatment, and survivorship.The OncologyNurseNavigatorwillassist the physician and staffwhileworking todeliversuperiorpatient experience.
PrimaryDuties &Responsibilities:
Act asthe primarypoint of contact for prostate cancer patients, theirfamilies, theUrologist, and internal and external care providers.
Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs and communicate any concerns to the Urologist.
Assist the patient and office staff in scheduling tests, procedures, appointments, and treatments.
Help develop an appropriate treatment plan in consultation with the Urologistthat is understandable and feasible forthe patient.
Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on a multidisciplinary care plan.
Assist patientsin overcomingbarriers to care (transportation, financial counseling).
Refer patients to supportive care services such as dieticians, social workers, and financial counselors.
After consultation with the Urologist, help eligible patients access appropriate clinical trials.
After consultation with the Urologist, educate patients about appropriate new technologies.
Direct patients and families to available community resources and supportive services.
Coordinate discharge planning with theUrologist and office staff.
Participate in multidisciplinary consultations/conferencesand document recommendations.
Participate in prostate cancer disease planning and help ensure compliance.
Assist in the development of patient education materials, clinical pathways, and qualitystudies.
Work with the Urologist, Radiation Oncologist, and office staff tocoordinate care andensure timely appointments, results reporting, and communication of patient care and follow-up, which includes internal and external care providers.
Promote community education and increase prevention awareness and early detection screening programs.
Monitor and report on outcomes measures toAdvanced ProstateCancerPhysician Champion, Prostate Cancer Patient Navigator, if applicable, and Program Administrator, if applicable.

Qualifications:
Bachelor9s degree ina healthcare-related field preferred,exceptin MD.
MD Only:Registered Nurse (RN) License: Must hold a current and active RN license inMD.
Experience working in a medical or healthcare settingpreferred.
Extensive knowledge of healthcare.
Urology experience preferred but not essential.
Knowledge, Competencies &Skills:
Excellent customer service skills.
Attention to detail and willingness to listen and learn.
Proficient knowledge of medical terminology.
Strong communication skills, both verbal & written.
Proficient computer software and database skills.
Comfortable working in a fast-paced environment.
Very comfortable asking probing questions to patients.
Must demonstrate a caring, supportive, compassionate, andpatient-centeredattitude.
Maintain HIPAA compliance.
Multitasking and proactive problem-solving.
Ability to communicate in a culturally diverse environment.

Must have the ability & willingness to travel to othersites as necessary within ageographic region.
Physical Requirements for theJob:
Regularly required to sit and stand for extended periods.
Involves standing, sitting, walking, bending, stooping, and twisting.
Requires full range of body motion, including manual and finger dexterity and eye-hand coordination.
May be required to help transfer a patient.
Occasionally lifts and carries items weighing up to40 lbs.
Job Type:Full-Time
Pay Range: $80,000 - $100,000 annually
Actual compensation offered to candidates is based on work experience, education, skill level,and geographic location. Compensation may vary depending on the state or region in which theposition islocated,in accordance withapplicable laws.
