Home Health Occupational Therapist for a
13-week, full-time contract. In this role, you'll provide personalized therapy that helps patients regain their independence and safely perform everyday activities in the comfort of their homes.
This is a contract position requiring at least one year of home health experience. The selected OT must be confident, independent, and prepared to manage patients with limited orientation.Contract Details:- 13-week, full-time contract

- Local travel throughout Charleston, Berkeley, and Dorchester Counties
Responsibilities:- Provide occupational therapy to patients who have difficulty performing daily activities due to an illness or injury.
- Perform comprehensive assessments, identify functional deficits, establish therapy goals, and document patient progress.
- Develop and implement individualized treatment plans based on each patient's needs and goals.
- Work independently while collaborating with the home health team, which may include RNs, LPNs, CNAs, HHAs, PTs, and SLPs.
- Help patients improve fine motor skills, strength, mobility, and coordination.
- Recommend adaptive equipment, including grab bars, shower chairs, and other supportive devices.
- Assess fall risks and implement strategies to improve safety within the home.
- Educate patients and families about the plan of care, exercises, goals, safety, and self-care.
- Complete accurate, thorough, and timely clinical documentation.
- Provide services in accordance with agency policies and all applicable regulations governing the scope of practice.
Qualifications:- Graduate of an accredited Occupational Therapy program.
- Current, active Occupational Therapist license in good standing in South Carolina.
- At least one year of occupational therapy experience in a home health setting is required.
- Must be able to work independently and transition into the role with limited orientation.
- Strong documentation and time-management skills.
- Tech-savvy and proficient in navigating electronic medical record systems.
- Knowledge of state and federal home health regulations.
- Strong clinical judgment, interpersonal skills, and problem-solving abilities.
- Current CPR certification.
- Valid driver's license, reliable transportation, and automobile insurance.
- Compassionate, dependable, and resourceful.
