In this role, you'll combine clinical nursing expertise with education to help patients and caregivers understand diagnosed conditions, treatment plans, medications, and healthcare technology. Working closely with an interdisciplinary team, you'll identify individual learning needs, develop personalized teaching plans, and evaluate readiness to support a safe transition from the hospital. As a Nursing Professional Development Practitioner, a typical day may include:
  • Assessing the individual learning needs of patients and families managing medically complex conditions and treatment plans
  • Developing individualized teaching plans that guide patient and family education throughout hospitalization and preparation for discharge
  • Providing clear, comprehensive education regarding diagnosed illnesses, treatment plans, medications, and healthcare technology requirements
  • Partnering with patients and caregivers to build the knowledge, skills, and confidence necessary to safely participate in care following discharge
  • Collaborating with nurses, physicians, therapists, and other interdisciplinary professionals to identify learning needs and coordinate patient and family education
  • Adapting teaching methods based on individual learning needs, abilities, and the complexity of each patient's care requirements
  • Evaluating patient and family understanding and competency to determine whether educational goals have been successfully achieved
  • Assessing the effectiveness of teaching strategies and learning outcomes and modifying education plans when additional support is needed
  • Promoting continuity between inpatient care and the patient's transition home by helping ensure families are prepared for ongoing care responsibilities
  • Providing direct nursing care when needed while maintaining a strong focus on education, competency development, and patient and family readiness
