Full Job Description
Role Summary

In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans.

Key Responsibilities

  • Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions.


  • Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies.


  • Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations.


  • Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting.


  • Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans.


  • Order and interpret labs, imaging, and other diagnostics; adjust treatment based on results and patient goals.


  • Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations.


  • Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health.


  • Document promptly and accurately in the EMR, including clear care plans and communication with the broader care team.


  • Participate in case reviews/huddles with the Home Health and medical teams to review panel outcomes and refine protocols.


What We're Looking For

  • Licensed Nurse Practitioner (Family or Adult-Gerontology) in Maryland, with prescriptive authority (or eligibility).


  • Minimum 2-3 years of experience in primary care, home-based care, wound care, or similar high-acuity outpatient setting.


  • Comfort working independently in the field while collaborating tightly with an interdisciplinary team.


  • Valid driver's license and reliable transportation for house calls within our service area.
