The Denver Hospice has an immediate opening for a Nurse Practitioner.

Location: Denver, CO

Status: Full-time

Schedule: Monday-Friday, 8-5, on-call rotating basis

PAY RANGE: $117,500 - $138,091 annually

SUPPLEMENTAL PAY: Based on position, schedule and/or availability: Paid Mileage/Shift Differentials/Stipends

MILEAGE AND EXPENSE REIMBURSEMENT: Sixty-two and a half cents per mile - one of the highest in the industry!

REWARDING WORK YOU WILL DO:

Responsible for the evaluation and treatment of patients in the hospice, palliative and/or home health programs. The Nurse Practitioner functions as a member of the Interdisciplinary Team/Group (IDT/IDG) and collaborates closely with the Medical Director/Chief Medical Officer, physicians, nurses and other health care professionals.

  • Performs physical exams and conducts consultations to evaluate the patient's health status, including relevant health and medical history and current physical or psychosocial complaints; formulates differential diagnoses based on the information obtained.
  • Determines clinical appropriateness of hospice, palliative and/or home health admissions, transfers from palliative care to hospice and discharge of hospice or palliative patients as needed. Provides appropriate recommendations of care such as hospice, rehabilitation, ongoing aggressive and/or palliative care. Collaborates with other healthcare providers and agencies to provide and coordinate services for patients and families/caregivers.
  • Assesses the individual needs of the patient, including pain and symptom management, goal setting, clarification of treatment options, advanced care planning, illness and end-of-life issues. Participates in the development of the patient's Plan of Care (POC) in IDT/IDG meetings, facilitating discussions and clarification of patient and family goals.
  • Writes orders, prescribes medications and other treatment modalities for symptom control; communicates recommendations to referring provider and nursing staff.
  • Orders and interprets laboratory and diagnostic tests relative to the patient's needs.
  • Conducts face-to-face visits with hospice patients at time of recertification according to statutory requirements.
  • Prepares and maintains timely and accurate patient records to support sound medical practice and coding for reimbursement for services provided.
  • Provides ongoing support to clinical staff.
  • Works with HIM Department to ensure medical documentation is authorized/approved as required.


WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Minimum Education: Graduate from a nationally accredited educational program as an advanced practitioner of nursing, or adult, family or geriatric nurse practitioner.
  • Minimum Experience: One to two (1-2) years of experience as a Nurse Practitioner; experience in Hospice, Internal Medicine, Family Practice, Pediatrics, Oncology or related field of medicine preferred .
  • Required License:
    • Current Colorado RN and APN/NP licenses.
    • Board certified as Adult or Family Nurse Practitioner by the AANP or ANCC.
    • Valid Colorado state driver's license.
    • Prescriptive authority and DEA number.
  • Current CPR certification. CPR must be provided and/or obtained within 14 days of hire.


Additional Qualifications:

  • Must be able to obtain hospital privileges at all local hospitals.


PHYSICAL REQUIREMENTS:

  • Ability to lift minimum of 50 lbs.
