Description

As a Nurse Practitioner or Physician Assistant on our team, you'll be at the forefront of this mission. You'll collaborate directly with primary care providers to manage acute and chronic pain, ultimately preventing functional decline and reducing hospital readmissions. Your role is crucial in assessing opioid-related risks and implementing effective strategies, such as utilizing risk assessment tools and patient education.

Responsibilities
• Deliver compassionate, patient-centered care by managing acute and chronic pain in skilled nursing and assisted living facilities throughout the Twin Cities metro area.
• Collaborate with primary care providers and interdisciplinary teams to develop comprehensive pain management plans.
• Conduct thorough patient assessments to identify pain sources and tailor individualized treatment strategies.
• Utilize evidence-based practices to prescribe medications responsibly, ensuring safe and effective pain relief.
• Educate patients and their families on pain management techniques, fostering understanding and adherence to treatment plans.
• Monitor and evaluate patient progress, adjusting care plans as necessary to optimize outcomes and enhance quality of life.
• Implement risk assessment tools and utilize prescription drug monitoring databases to mitigate opioid-related issues proactively.

Qualifications
• At least 1-2 years as a Nurse Practitioner (NP) or Physician Assistant (PA).
• Master's degree or higher.
• Experience working in a skilled nursing facility (SNF) strongly preferred.
• Pain management experience strongly preferred but not required.
• Ability to build and foster trust-based relationships with patients, families, and diverse healthcare teams.
• Experience reviewing and maintaining up-to-date medical records in EHR systems.
• Valid driver's license and auto insurance.
• Reliable transportation to travel between care facilities in and around the Twin Cities metro (Minneapolis, St. Paul, and the surrounding suburbs).
