Full Job Description
Providence Medical Group (PMG) is seeking an experienced nurse practitioner or physician assistant to join a Primary Care team at Mary's Woods, a premier senior living community in beautiful Lake Oswego, Oregon.

This unique opportunity offers the chance to provide comprehensive, relationship-centered care to an aging patient population while working alongside a highly collaborative, multidisciplinary team. You'll partner with Family and Internal Medicine physicians, advanced practice clinicians, a behavioral health provider, clinical pharmacist, case management team, and experienced clinical support staff in a value-based care environment focused on improving patient outcomes and quality of life.

Located just minutes from Portland, Mary's Woods serves patients from Lake Oswego, West Linn, Oregon City, and surrounding communities, offering an exceptional quality of life in one of the Pacific Northwest's most desirable locations.

Position Highlights:
  • Full-time position (1.0 FTE)
  • Monday through Friday schedule with daytime clinic hours
  • Flexible 4- or 5-day workweek
  • Non-paneled role focused on team-based care
  • Combination of direct patient care and indirect patient management, including in-basket support
  • Opportunity to care for medically complex geriatric patients in a supportive, collaborative setting

  • Nurse Practitioner or Physician Assistant with experience in Primary Care
