Nurse Practitioner-Primary CareFull-Time11:00am-11:00pm or 3:00pm-11:00pm ShiftIn this role working alongside caring, dedicated, and experienced Medical leadership, the Nurse Practitioner will report to the Medical Director, or the supervisory Attending Physician, and is responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the health care team.
Some key responsibilities of the position include examining residents and delivering quality care in varied settings including subacute, long term, rehab and outpatient care, as well as assisting in resident/patient/family education and re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts.
Position Qualifications- Two (2) or more years of experience in a sub-acute or long term care facility
- Graduate of an accredited nurse practitioner program
- Active license as an NP in New York State, with prescriptive privileges
- NYS Board Certified is required
- Proficient in SigmaCare or another EMR (Electronic Medical Record) software
- Prior experience with the geriatric population is a plus
