Key Responsibilities:
- Manage a personal panel of patients and/or co-manage patients with other physicians in the practice.
- Build strong, trusting relationships with patients, focusing on holistic care and long-term wellness.
- Collaborate with interdisciplinary teams to ensure seamless and comprehensive patient care.
- Maintain accurate and thorough documentation in compliance with regulatory and organizational standards.
- Continuously improve skills and integrate evidence-based practices into clinical decision-making.
Qualifications:
- Licensed provider with an active, unrestricted license in Massachusetts.
- Demonstrated commitment to patient-centered, high-quality care.
- Strong communication and collaboration skills.
