JOB TITLE: Provider (Nurse Practitioner)

POSITION SUMMARY

The Provider is responsible for delivering expert, personalized medical care while serving as a trusted partner in each patient's health journey.

This role combines clinical expertise with patient education and individualized treatment planning, ensuring patients feel informed, confident, and supported in achieving their health goals.

PRIMARY RESPONSIBILITIES

Clinical Care & Treatment Planning
  • Conduct comprehensive assessments and physical exams

Patient Education & Partnership
  • Educate patients on treatment options, protocols, and expected outcomes
  • Provide clear guidance on lifestyle, wellness, and long-term health optimization

Patient Experience & Communication
  • Deliver a high-touch, personalized care experience
  • Use clear, direct, and reassuring communication
  • Build trust and long-term relationships with patients

Team Collaboration

This role requires flexibility, adaptability, and a team-first mindset. All team members are expected to support clinic operations and patient needs as they arise, within the scope of their role and licensure.
  • Partner with PRCs, PEMs, and clinical staff to ensure seamless care delivery
  • Support a unified, team-based approach to patient experience
  • Contribute to a positive, collaborative clinic environment
