Job Description

A Nurse Practitioner is a professional nurse who possesses the advanced education, knowledge, clinical skills and experience necessary to provide expert nursing care in the area of specialization. The Hereditary Hemorrhagic Telangiectasia (HHT) Nurse Practitioner has in-depth knowledge in the neurosciences, vascular disease, hemorrhage management, and genetic evaluation. Under the direction of the Program Directors of the UCSF HHT Center of Excellence, the HHT Nurse Practitioner has responsibilities in clinical practice, education, consultation, research, utilization review and leadership within the HHT Center of Excellence and the Medical Center.

HHT is a multisystem genetic disorder occurring in about 1/5000 individuals. The vascular malformations that accompany HHT can result in brain hemorrhage, pulmonary hemorrhage, epistaxis (nosebleeds), gastrointestinal bleeding, and chronic anemia. The HHT Foundation certifies hospitals as centers of excellence based on development of multidisciplinary teams of physicians to care for the various manifestations of the disease.

Responsibilities

HHT Center of Excellence

Pre-hospital/pre-procedural patient evaluation

• Educates patients and family members on disease process, treatment alternatives, pre-procedural preparation, hospitalization, and discharge.
• Receives and evaluates patient reports from sending institution and confers with UCSF/ZFGH physicians regarding patient condition and care requirements.
• Plans comprehensive care with other health providers.
• Confirms that all lab reports, medication lists, informed consent forms, Humanitarian Device Authorizations have been received and reviewed.

Outpatient and Inpatient Care

• Responsible for overall administration of HHT clinic as part of HHT Center of Excellence (CoE).
• Coordinates HHT clinic appointments, imaging requests, and scheduling of appointments with subspecialists depending on patient needs.
• Coordinates anesthesia for scheduled procedures.
• Assists staff and physicians with insurance authorization process as needed.
• Performs follow up phone calls for HHT clinic patients for routine HHT-related health conditions as well as for post-procedural care.
• Performs inpatient daily rounds with or under the supervision of interventional radiology and interventional neuroradiology physicians as needed.
• Performs ongoing assessment of HHT patient progress, changes, and condition, reporting to HHT CoE physicians as necessary.
• Responds to patient care requests by phone, appropriately triages patient care, and seeks advice from HHT CoE physicians as needed.
• Maintains communication with care team members and assists with patient care as needed.
• Screens patients for clinical trials, coordinates data collection, analyze, draft and revise research protocols.

Qualifications

Required Qualifications:

• Graduation from a California Board of Registered Nursing approved or verified school of nursing with a master's degree in nursing or a Doctor of Nursing Practice (DNP).
• 1-3 years experience as nurse practitioner in Neurology, Neurovascular, Neurosurgery or Neuro IR, hematology, OHNS, and/or HHT.

Preferred Qualifications:

• Case management experience
• Experience managing clinical research protocols and working with hospital human subjects research committees
• Knowledge of ethical conduct of Human Subjects Research

Required Licensure/Certifications:

1. California Registered Nurse (RN) License without restriction
2. California Certified Nurse Practitioner (NP) without restriction
3. California Nurse Practitioner Furnishing Number including Schedule II furnishing privileges
4. NP national board certification from an accepted certifying agency aligned with the patient population and clinical role
Adult Hybrid Inpatient and Outpatient (ACNP-BC, AGACNP-BC, ACNPC-AG, ACNPC). See Addendum A below for more detail.
5. California Driver License (if required for position)
6. Drug Enforcement Administration (DEA) Registration with Schedules 2, 2N, 3, 3N, 4, and 5 registered to a California address
7. American Heart Association (AHA) Basic Life Support (BLS) certification
8. Additional required AHA certifications: ACLS
9. Additional required certifications (such as ENLS): None

UCSF Medical Center requires all Advanced Practice Providers (APPs) to be credentialed through its Office of Medical Affairs and Governance (OMAG) to practice and function in their clinical role. This applies to both adult and pediatric APPs in the inpatient and outpatient clinical settings. Credentialing takes between 60-90 days and must be completed prior to the first working day.

Salary Information

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
